Appointment Checklist
To ensure we spend our time on what’s most important, you. We do require a  couple prepatory tasks including:

  1. Download and print our patient intake form
  2. Complete and return to our office prior to your visit if possible
  3. Please have all records, labs and imaging relating to your visit faxed to our office at 480-314-0628
  4. If anything changes, please contact us as soon as possible

Any of your information received in advance is helpful in accessing your needs.

    Important to Note
    Our office does not accept any health insurance.  We are a fee-for-service office and payment is due at time of services.  We will give patients a copy of their charges for submission, but do not assist with any insurance reimbursement.
